I worked as a Hospital Unit Clerk in a transition unit, helping coordinate paperwork and communication for patients moving into long-term care.
Every day I watched families arrive overwhelmed. They were trying to understand medical instructions, complete government paperwork, locate missing documents, and make major decisions for their parents — all at the same time.
Then later, I found myself on the other side of the system.
When I became the primary caregiver for my own mother, I experienced those same challenges firsthand. Even with my insider knowledge of how hospitals work, I still found the process confusing and stressful.
That’s when something became very clear to me.
Families aren’t failing the system — the system is simply too complicated without a clear structure.
So I began organizing everything I had learned into a simple framework designed to help families navigate parent transitions with clarity.
I call it The Parent Transition Master Suite.
